Emergency Remote Services

Angeles Del Sol ADHC will provide the following services to assist all of our participants at home if an emergency situation makes you unable to receive in-center benefits.

  • Regular Telehealth call: Including wellness check                                            Covid-19 check & services                                          Nursing services                                                          SW support & referral

  • Telephonic LCSW mental health services

  • Telephonic dietitian counseling

  • Nutritional meal delivery

  • Therapeutic activity packet and Zoom classes

  • Other essential supply delivery services: including food supplies, PPE, mediation refill and delivery, food bank delivery, etc. )

  • Other services: please call us to get more information. 

Two types of Emergency that may qualify you for ERS:1. Public Emergencies, such as state or local disasters, regardless of whether formally declared;

2. Personal Emergencies, such as illness, injury, crises, or care transitions.

Nursing Home

NURSING & PERSONAL CARE

We provide onsite professional nursing services​, which include general health monitoring and assessment, medication management, communication with participant's personal health providers; personal care services; skilled nursing services; assist participants in obtaining medical services; health education, and counseling when needed.

Healing Hands

SOCIAL WORKER SERVICE

Social workers will provide supervision, assessment, and monitoring of the participant's psychosocial status to maintain and improve the participant's emotional wellbeing; conduct problem-centered support groups to address psychosocial issues; case management and care coordination services are also provided to help participants receive needed resources and maximize independence. 

Diet Plan

NUTRITIONAL MEALS & COUNSELING*

Nutritional meals are provided based on physician’s order and Registered Dietitian’s nutritional assessments and counseling. The center will also tail to the participant’s needs and preferences.

Crafts Activity

RECREATIONAL ACTIVITY

Participants can enjoy various different activities of their own preferences on different days of the week for social engagement, cognitive stimulation, exercise, and entertainment. We also provide customized group or individual activities for those participants who are frail or have a cognitive functioning impairment.

TRANSPORTATION

We provide round trip transportation for our members to commute to our center. Our service area includes but is not limited to Diamond Bar, Pomona, Walnut, Chino, Chino Hills, Covina.

Physical Therapy Session

PHYSICAL THERAPY

We have a licensed physical therapist who provides personalized exercises and treatments to maintain mobility and strengthen endurance.

Senior Therapy

OCCUPATIONAL THERAPY

We have a licensed Occupational Therapist who performs assessments, develops an individual care plan, provides therapy and education that promotes ongoing support in activities of daily living.

PSYCHOLOGICAL/PSYCHIATRIC THERAPY

A licensed clinical social worker (LCSW) will provide education for emotional and mental health, counseling for personal, family, and social issues.

SPEECH THERAPY

When participants need help with swallowing, simple and complex verbal expression, auditory processing, voice changes, and poor speech intelligibility, our licensed speech therapist will assess and treat participants with their scope of practice.
